Mission

The Lanark, Leeds and Grenville Ontario Health Team (LLG OHT) recognizes the challenges in providing health services across a broad geographical area, which is predominantly rural. With this in mind, our aim is to build an integrated system of care that provides equal access to the services and supports needed to build and maintain the health of all the residents in our region. 

We provide leadership through the collective voice of our members. Together, we engage partners to achieve optimal care and health outcomes for patients, care partners, community members and providers.

Vision

A comprehensive, accessible and integrated system of care, which improves the well-being of all individuals, families and communities in the Lanark, Leeds and Grenville region.

Patient declaration of values

The LLG OHT Lived Experience Advisory Network (LEAN) has adopted the Ministry’s Patient Declaration of Values. The five values are:
