The Primary Care Network (PCN) is a network of doctors and nurse practitioners working together to improve care in our communities.
The PCN's goal is to improve and better coordinate patient access to primary care as it is often the first point of contact most people have with the health system.
The PCN works together to support the design of programs and services to best meet the needs of residents within Lanark, Leeds and Grenville, which may vary from area to area. This flexibility to develop local solutions to local health issues sets our PCN approach apart.
Goals of a PCN include supporting the LLG OHT’s priority projects, such as Connecting Residents to Primary Care, and Connecting Mental Health and Addictions Services to Primary Care.
