Ensuring all residents have a family doctor or primary care home

LLG OHT’s second change initiative focuses on working towards ensuring all residents have a family doctor or primary care home.

This focus area attempts to maximize health outcomes, improve equitable access, and works towards ensuring all residents are attached to a primary care home. This will lead to improved health services and more efficient use of health system resources. The LLG OHT will complete the following work in this focus area:

• Identify primary care assets across the region 
• Identify potential additional capacity at a primary care model level 
• Implement communication strategies to connect unattached patients to available/appropriate primary care capacity 
• Create an LLG OHT primary care health human resources plan 
• Develop an OHT primary care recruitment strategy
• Coordinate local intake process
