Full Ontario Health Team partners gathered, support committees and project teams formed

CDMA finalized for Ontario Health Team partners, year 1 priorities identified

Approval and sign off of CDMA attestation form by all team members

Create a Primary Care Network (PCN)

We believe that primary care (your day-to-day healthcare provider, such as your family physician) is the foundation of our local health system. We intend to create a local primary care network for the region.

To maximize health outcomes, we will work to ensure that all residents are attached to a primary care home (i.e., have a healthcare professional they can see for their day-to-day health care needs, such as a family physician) and improve equitable access to health services.
