The Lanark Leeds and Grenville Ontario Health Team is pleased to announce that Morgan Hawn has been appointed as its new Executive Lead. Hawn joins from ConnectWell Community Health, where she is currently the Director of Primary Care.
“I am very excited to provide leadership and support to my colleagues and partners within the Lanark Leeds and Grenville Ontario Health Team,” shared Hawn. “Now more than ever, our healthcare and social service systems need to continue to collaborate to improve the overall patient and provider experience, strive to integrate services and provide seamless transitions of care. I would like to take this opportunity to recognize the effort and dedication of all OHT partners towards improving the system and I am excited to continue building on this great work.”
The Lanark Leeds and Grenville Ontario Health Team is a partnership of more than 50 health, community and social service organizations across the Counties of Lanark, Leeds and Grenville. By putting the patient, resident, family and care partner voice front and centre, the Lanark Leeds and Grenville Ontario Health Team is committed to improving health care for everyone in the region.
