The Ministry of Health introduced Ontario Health Teams to build a connected health care system centered around patients, families and caregivers. By working together, Ontario Health Teams will strengthen local services, making it easier for patients to navigate the system and transition between providers. Officially approved to proceed with development in November 2020, the Lanark, Leeds and Grenville Ontario Health Team (LLG OHT) represents a partnership among members/partner health system organizations/providers to develop a local model that supports a continuum of care with providers in the United Counties of Leeds and Grenville, and the County of Lanark area.
The LLG OHT is one of over 50 Ontario Health Teams established across Ontario. Locally, LLG OHT is a partnership of more than 50 health, community and social service organizations across the Counties of Lanark, Leeds and Grenville who are working together to improve health care for everyone in the region. We are committed to partnering with patients, clients, residents and care partners in our communities to deliver more coordinated, seamless care for everyone we serve. This begins by putting the patient, resident, family and care partner voice front and centre.
During the application process, OHTs are required to select projects to start this transformation. In consultation with many partners, the LLG OHT identified two key challenges faced by residents: access to a primary care provider and access to mental health and addictions services. These serve as our priority projects.
